Victims’ Rights Request/Waiver Form – Annual Supply Order

*ATTENTION: As of 2025, the AGO will no longer be supplying agencies with printed versions of the Victims’ Rights Request/Waiver Form (R/W) form booklets and will be providing the updated versions of the R/W form to agencies in electronic format. If an agency requires or prefers a printed version of the form booklets, they may print and pay for the R/W form booklets independently. Please complete the order form below. Even if your agency is using a custom version of the R/W form, please complete this order form. As a reminder, if your agency uses a custom form, it must be approved by the Attorney General’s Office per A.R.S. §§ 13-4405(E)(5) and 8-386(E)(5).

Along with the order form submission, your review of the form and the resources listed is critical to the accuracy of this information. We invite your comments on the form, including corrections, updates and additions to the phone numbers listed on the Information and Resources page. We cannot have accurate information for victims without your help. We will review and consider all comments and suggestions received. Please review the current version of the form applicable to your region of the State (linked on the online order form) and submit your comments with the submission of the order form.

The R/W Form is available on AZ TraCS! The use of AZ TraCS or the electronic PDF fillable version of the R/W form can save your officers time in the field, provide accurate and legible information to victims, external criminal justice partners, and community members, saves electronically in RMS, and is low cost to maintain. Our agency will use the AG's Offices' Victims' Rights Request/Waiver Form.
*A law enforcement agency that chooses to establish its own procedures, pursuant to A.R.S. §§13-4405(E)(5) and 8-386(E)(5), must do the following: (1) Develop procedures that conform to statutory intent; and (2) report its proposed procedures to the Attorney General’s Office for approval.
