Choice Home Warranty Settlement

This form relates to the 2026 settlement between Attorney General Kris Mayes and Choice Home Warranty. Choice Home Warranty customers who purchased a warranty over the phone between January 1, 2013 and December 31, 2025 for homes located in Arizona may eligible for restitution under this settlement. Please fill out the form below to be considered for restitution.

Not all customers will qualify for restitution. Eligibility to receive restitution and the amount received will be in the sole discretion of the Attorney General. Due to the nature of the settlement, restitution may not be immediately available to all claimants. Restitution will be paid on a rolling basis as it is received from the company. Claimants who submit false information will not be eligible to receive restitution. An inability to recall some of the information requested below will not automatically disqualify claimants from receiving restitution, and claimants who are unable to recall specific information requested are encouraged to indicate this in the comments section below.
